Challenge: Optimising Coordination and Communication in Surgical Blocks
As incredible as it may seem, many hospitals across Europe continue to manage critical, time-sensitive healthcare processes through outdated methods. Coordination among care teams still often relies on shouting down corridors or making endless phone calls to locate a porter, a piece of equipment, or a patient. Meanwhile, nurses spend a significant portion of their time manually recording data into files or fragmented information systems. This constant administrative burden diverts attention away from direct patient care, fuels frustration among professionals, and limits the hospital’s capacity to meet the growing pressure on healthcare demand. A radical transformation in the way hospitals operate is urgently needed.

This challenge becomes even more evident in one of the hospital’s most complex and demanding service — the surgical Unit. Within the surgical block, Hospital Clínico mobilises around 150 healthcare professionals working across 16 operating rooms: 12 dedicated to elective procedures, 3 to emergencies and 1 maternal. These professionals include surgeons, anaesthetists, nurses, porters, and cleaning teams distributed across admission, preparation, operating, and recovery areas. Efficient coordination between these highly interdependent roles is essential to ensure patient safety, maintain high-quality care, and keep waiting times under control. However, the lack of real-time visibility and digital integration often causes inefficiencies, delays, and unnecessary stress for all involved.


Another critical but frequently underestimated aspect is communication with patients’ relatives. This moment, occurring during surgery, is one of the most delicate and emotionally charged points in the entire process. When communication with families is inadequate or unstructured, it can substantially diminish satisfaction levels, increase complaints to hospital management, and, in some cases, erode trust in the surgical team. From an operational perspective, poor communication also creates a repetitive workload for clinical staff, as anxious relatives request updates in the absence of reliable, scheduled information. Improving this interaction is therefore essential not only for patient and family experience but also for staff efficiency and morale.
Recognising these challenges, the Hospital Clinico de Valencia is willing to pilot a solution designed to demonstrate how digital transformation can radically enhance surgical workflow efficiency and coordination. The main goal is to increase overall surgical performance, despite the technical constraints of an older hospital infrastructure. Lessons learned from this pilot will inform the design and organisation of the hospital’s new facilities, ensuring continuity in both technology and workflow innovation.


Specific objectives of the pilot include tracking deployment time (DT)—with a target of completing system integration in less than 12 weeks—while also measuring staff satisfaction through the Job Satisfaction Survey (JSS) and patients’ and families’ satisfaction via the Short-Form Patient Satisfaction Questionnaire (PSQ-18). The success criteria aim for an 15% improvement in both staff and family satisfaction levels compared to baseline data.
Ultimately, this initiative seeks not only to digitalise hospital operations but to redefine the surgical experience for patients, families, and professionals alike—making the Hospital Clínico Universitario de Valencia a leading example of human-centred innovation in the Valencian region.

Hospital Clínico Universitario de Valencia (HCLIV)

The Hospital Clínico Universitario de Valencia (HCLIV) is one of the major tertiary care institutions in the Valencian Community, Spain. Located in Valencia city center, it is affiliated with the University of Valencia and integrates medical assistance, teaching, and research. It also forms part of the health department known as Valencia – Clínico – Malvarrosa, together with its supporting facility, the Hospital de la Malvarrosa. The hospital is a referral center for several specialties and is one of the three public hospitals in the Valencian Community with an accredited Health Research Institute, INCLIVA.

The Hospital Clínico Universitario de Valencia (HCLIV) is one of the major tertiary care institutions in the Valencian Community, Spain. Located in Valencia city center, it is affiliated with the University of Valencia and integrates medical assistance, teaching, and research. It also forms part of the health department known as Valencia – Clínico – Malvarrosa, together with its supporting facility, the Hospital de la Malvarrosa. The hospital is a referral center for several specialties and maintains strong links with the INCLIVA Biomedical Research Institute.

INCLIVA is the Foundation for Research at the Clinical Hospital of Valencia, established in 2000 under the auspices of the Valencian Government, becoming the first foundation in the Valencian Community to be affiliated with a public hospital. Its purpose is to promote and facilitate scientific and technical research within the Department of Health of Valencia Clínico-Malvarrosa, led by Hospital Clínico as the main reference centre. Currently, INCLIVA has 54 research teams, 11 of them emerging groups, encompassing over 800 researchers.

The hospital’s capacity is reported to be around 538 installed, serving a total population of 360.000 people according to the latest public information from the 2024 Annual Report. The surgical infrastructure at HCLIV is extensive. Its surgical block contains 16 operating theatres distributed over two floors. 12 of these are multipurpose operating rooms dedicated to scheduled surgical procedures in different specialties. In addition, there is 1 operating room specifically for local procedures, 2 operating rooms permanently available for emergency surgery, and 1 dedicated obstetric operating room located in the maternity area, which also includes four delivery and dilation rooms. Post-operative support is provided by a recovery unit with twelve beds for low-risk patients, a post-anesthesia recovery unit with ten beds for more complex cases, and a reanimation and critical care unit with twelve beds for surgical and trauma patients. Ambulatory surgery is supported by a day-care unit with approximately forty beds.

Hospital Clínico registered nearly 23.000 scheduled surgeries and around 4.300 emergency interventions. It handles a high volume of activity beyond surgery, including tens of thousands of emergency visits and outpatient consultations each year.

MYSPHERA SL

MYSPHERA originated in 2012 as a spin-off of TSB Solutions Group, closely linked to the Universitat Politècnica de València. The company has spent more than 10 years developing real time location solutions for hospitals. In the early stages, MYSPHERA was able to grow providing patient tracking solutions focused on delivering applications that show patient location in real time. With this approach, the company has grown gradually, reaching around 50 hospitals (customers). However, growth in recent years has slowed down, and the company needed to pivot. With this in mind, the MYSPHERA came to the realization that the key value for a hospital is not about showing patient locations, but how this real time location information can be used to automate and optimise hospital processes.

A tipping point was reached when MYSPHERA was granted the EIC PATHMAKER project in 2021, which provided a €1.810.550 grant and secured the support of the EIC for a second venture capital (VC) investment. The SME was able to develop a cutting-edge solution to implement fast pathways to optimise hospital processes. The aim was to catapult the company’s growth through the new value brought by PATHMAKER. Moreover, the company developed a channel strategy to internationalise the solution including key partners such as Siemens, Air Liquide and Telefónica and was acclaimed in various European competitions, being highly commended at the European Design Healthcare Awards in 2023, and finalist in the Value-Based Healthcare Prize in 2023 and 2024.

Solution

PATHMAKER is an advanced patient flow management solution designed to improve coordination and efficiency in hospital surgical units. By combining real-time location technology with a powered orchestration platform, PATHMAKER enables healthcare teams to work more efficiently, reduce delays, and improve patient experience throughout the surgical process.

At the core of the system is MYSPHERA’s Real-Time Location System (RTLS), which uses Bluetooth Low Energy (BLE) wristbands to track the movement of each patient across key surgical areas — from admission and preparation, to surgery, recovery, and discharge. This continuous flow of data is processed by the PATHMAKER orchestration engine, which provides real-time support and task allocation for all teams involved.


This real-time coordination reduces stress, eliminates inefficiencies, and enhances the surgical team’s performance giving real time information and notifying the appropriate staff — such as porters, nurses, or cleaning teams — via dashboards or mobile devices to trigger the necessary actions.


This orchestration ensures, for example, when operating rooms are prepared and available, patients are moved at the right moment. Surgical teams can focus on care rather than logistics. PATHMAKER operates continuously across all surgical workflows, reducing unnecessary waiting times, avoiding bottlenecks, and contributing to smoother, more predictable operations.


Additionally, it allows hospitals to share status updates with patients’ families, offering information through the whole process.
