Lombardia Regional Cerebral Stroke project

Cerebral stroke is a major public health concern, with ischemic stroke accounting for approximately 80% of all cases. In Italy, its incidence fluctuates between 106 and 313 cases per 100,000 inhabitants annually, making it the third leading cause of death and responsible for 10-12% of total fatalities. Lombardy region, home to 10.4 million people, experiences approximately 18,000 strokes annually. Delivering effective stroke care in this region presents significant challenges due to the time-critical nature of the disease and the vast distances between local hospitals and specialized stroke centers.

To address the need for more efficient stroke care, Regione Lombardia has structured a specialized stroke network consisting of designated Hub centers and associated Spoke hospitals, ensuring a streamlined approach to acute stroke management. As part of its commitment to innovation, Regione Lombardia is piloting cutting-edge AI technology to enhance stroke diagnostics and treatment workflows. A key initiative in this effort is the EIC-funded Stroke AI Pilot, which has selected Nicolab’s StrokeViewer as the preferred AI solution for the EIC InnoMatch call.

Through this pilot, StrokeViewer will empower clinicians by:
-Enhancing stroke diagnostics with AI-driven CT (angiography) and CTP analysis, enabling faster and more accurate decision-making.
-Improving regional stroke coordination through real-time image sharing, facilitating seamless collaboration between hospitals.

This initiative serves as a proof of concept for a regional AI-powered stroke platform, demonstrating the feasibility of scaling AI-driven solutions to improve stroke care. If successful, this model can be expanded across Lombardy and serve as a blueprint for AI integration in stroke networks across Europe.

Regional healthcare objectives in Lombardy are evolving with a review that places greater emphasis on prevention rapid actions for time-dependent pathologies.

The use of digital tools and data transparency are at the heart of planning for a more efficient, equitable healthcare system focused on citizens’ needs.

The Lombardy Region has devoted particular attention to stroke management as part of its 2025 healthcare planning. The measures include greater integration between hospitals and the local area, with the establishment of specialist networks for cerebral stroke that guarantee rapid and coordinated diagnostic and therapeutic pathways. Emergency and urgent care departments are being upgraded, strengthening stroke units to ensure effective treatments such as thrombolysis and mechanical thrombectomy.

The use of innovative technologies and continuous training of healthcare personnel are key to improving clinical outcomes and reducing intervention times.

The implementation of regional digital healthcare faces several challenges: the use of obsolete technologies that slow down modernization and increase vulnerability to cyberattacks; the security and privacy of healthcare data; the difficulty of integrating and making existing information systems interoperable; the need for adequate training for healthcare personnel; cultural and organizational resistance to change; and patient involvement and acceptance. In addition, it is crucial to ensure economic sustainability and regulatory updates to effectively support digitization. Overcoming these challenges is essential to improving the efficiency, quality, and equity of social and healthcare services.

With one-click access, physicians can securely share cases, forward patients, switch between hospitals, and coordinate care through integrated tools such as chat, screen sharing, and push-to-PACS.
